Limestone gothic-revival style mausoleum built on rectangular ground plan. Pitched stone roof having limestone eaves course, copings and raised barges. Coursed roughly dressed limestone walls having ashlar limestone angle buttresses to corners and engaged buttresses flanking doorway (east elevation). Inscribed limestone plaque over doorway (east elevation) reading "This tomb was erected by Mary Widow of Edward O’Grady Esq. of Stephens Green Dublin for him and their immediate descendants. 1838". Tudor-arched doorway to east elevation having a chamfered ashlar limestone surround. Replacement timber double-leaf doors with original ironmongery and lattice overlight.

The family burial tomb of Edward Smyth O’Grady and his wife Mary O’Grady (nee. Stamer). A prominent local family, Edward was a younger brother of Standish O'Grady 1st Viscount Guillamore.
Conservation works were carried out in 2012/2013 by Knockainey Historical & Conservation Society aided by generous grant funding from LEADER, The Heritage Council and The Follies Trust.
Works included re-pointing, consolidation and repair of the roof and buttresses carried out by Irish Natural Stone, Co Clare and a replacement timber door constructed by joiner Dick Carroll, Hospital, Co. Limerick.
